When you take an assessment, you will get a rubric score that follows this convention:
4 = Mastery: “I know it very well, and can show my thinking with multiple representations. I could teach it to someone else.”
3 = Proficient: “I understand it and I can do it by myself with few (calculation) errors.”
2 = Partially Proficient: “I can do it with some help, I need more practice to do on my own without errors. My work shows conceptual errors.”
1 = Approaching Proficient: “I can only do it with help. My work shows I can attempt the mathematics.”
If you do not show skills that match with a 3 or 4, and you would like to re-attempt the assessment, your teacher will explain the process for retakes. Since each assessment will be different, each retake will also be different. All work for the current chapter must be turned in in order to do a retake.

If you are unhappy with your test/quiz score follow the steps below:
1) Fill out a Retake Application (link)
2) Complete and turn in all homework from the chapter
3) Correct problems missed on the test or quiz and study for retake.
4) Get help for this process during math clinic or see your teacher for help.
5) Find out retake date/time from your teacher.
Note: New score replaces old score (even if it is lower).
